By submitting this form, the customer understands that 2-3 weeks must be given for design, production and delivery prior to the date needed.
By submitting this form, the customer understands that 2-3 weeks starts AFTER payment has been received. Delay in payment will cause a delay in delivery.
By submitting this form, the customer understands that this is a custom order and therefore there are no exchanges, refunds or returns allowed. All sales are final once artwork is approved.
By submitting this form, the customer understands that there is an additional $25 artwork fee, per uniform requested that will be added to the final invoice. This artwork fee includes up to three corrections/edits made by the customer. After that an additional $25 artwork fee will be applied and must be paid prior to production and shipping.
By submitting this form, the customer understands that the cost of shipping will be determined once final number of ornaments are calculated.
